    Community Health Needs Assessment Activities: 2021

    • The ACA requires all 501(c)(3) tax-exempt hospitals to conduct a Community Health Needs Assessment (CHNA) every three years, starting with the hospital's tax year beginning after March 23, 2012. The 2011 Schedule H included an optional section of questions on the CHNA process. This web tool includes responses for those hospitals voluntary reporting this information.       IT IS SMALL RURAL COMMUNITY WITH THE NEAREST REGIONAL HOSPITAL 45 MILES AWAY. WE HAVE SEVERE WINTERS THAT AFFECT TRAVEL OUT OF THE COMMUNITY PERIODICALLY. WE SERVE A MIX OF ELDERLY, LOW INCOME AND WORKING CLASS PATIENTS.
      PART VI, LINE 5:
      PROVIDED STAFF FOR LOCAL HEALTH DEPARTMENT COVID TESTING EVENT IN JULY 2020 AND PROVIDED STAFF FOR THE LOCAL HEALTH DEPARTMENT COVID VACCINE EVENT IN FEBRUARY 2021. THE HOSPITAL IS DONATING SPACE TO GREAT LAKES RECOVERY WHICH TREATS PEOPLE WITH SUBSTANCE ABUSE PROBLEMS.
